Ensures health information is properly recorded into medical records so practitioners can plan and evaluate health care provided to patients. Performs one or more of the following: coordination of duties to include electronic indexing, filing and distribution of medical records, retrieval and release of patient record. Specific responsibilities include scanning and indexing all records coming in by mail, fax and from other outside sources; obtaining records needed from other healthcare agencies prior to patient appointment; responding to requests for information from files according to established policies and procedures; processing all medical records request forms within North Carolina laws and MAHEC regulations regarding release of medical information for law offices, insurance companies, other medical offices or facilities and patient copies; entering data and correcting errors in EMR and Practice Management systems; mailing printed lab results to patients as requested by physicians; monitoring and handling items on queue list in a timely manner; verifying presence of dictated EMR notes; answering calls from Clinical staff pertaining to medical records; complying with federal HIPAA (45 CFR), and 42 CFR regulations, practice policies for the privacy and security of patient information, and maintaining appropriate documentation of access to medical records. The role requires effective communication skills, decision making, healthcare knowledge, interpersonal skills, adherence to organizational values, and problem solving abilities. Computer skills in Microsoft Office including Word, Excel, PowerPoint, and database applications are required. Spanish speaking skills are preferred. Minimum qualifications include a High School Diploma or equivalent, at least 2 years of relevant experience in a medical office or hospital setting, or a comparable equivalent of combined education and experience. Preferred qualifications include RHIT, RHIA Certification, Associate or Bachelor Degree in related Healthcare field. Regular attendance on-site is essential with typical business hours Monday – Friday, 8:00 am to 5:00 pm, 40 hours per workweek, with occasional weekend, holiday, or evening coverage required.
